Complex & Community Care

Our clients range from people with spinal injuries, acquired and traumatic brain injuries, physical disabilities or learning disabilities through to mental health issues and those requiring palliative or end-of-life care. Our main objective is to support these clients to remain at home and live as independently as possible.

We assess the needs of each individual and, in partnership with multi-disciplinary teams, develop a care plan with them, which is reviewed regularly. We look at all aspects of their daily living as well as future aspirations, goals and outcomes. Our plans can involve a variety of specialists including:
